Sutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing Non-Invasive Cardiology program in San Francisco, California.
This position works closely with Non-Invasive Cardiologists to provide comprehensive cardiovascular care across the outpatient setting. The NP/PA will play a key role in evaluating and managing patients with a broad range of cardiac conditions, supporting diagnostic workups, coordinating longitudinal care, and promoting improved cardiovascular outcomes through patient education and evidence-based treatment strategies.
The NP/PA will partner with physicians and multidisciplinary team members to deliver high-quality, patient-centered care, improve access to specialty services, and support continuity of care throughout the patient's cardiovascular journey. Opportunity Details:
- Conduct comprehensive evaluations and follow-up visits for patients with cardiovascular disease in the outpatient setting
- Partner with Non-Invasive Cardiologists to manage acute and chronic cardiac conditions
- Perform detailed cardiovascular assessments, order and interpret diagnostic testing, and develop treatment plans in collaboration with supervising physicians
- Evaluate and manage patients with conditions including coronary artery disease, heart failure, arrhythmias, hypertension, hyperlipidemia, and valvular heart disease
- Review and interpret diagnostic studies including electrocardiograms (EKGs), echocardiograms, ambulatory rhythm monitoring, stress testing, and cardiac imaging results
- Support timely follow-up after emergency department visits, hospitalizations, and cardiovascular procedures
- Provide patient education regarding cardiovascular disease prevention, medications, risk factor modification, and lifestyle management
- Promote medication adherence and patient engagement to improve long-term cardiovascular outcomes
- Collaborate with cardiologists, nursing staff, primary care providers, and other specialists to ensure coordinated care
- Conduct in-person and telehealth visits as appropriate
- Manage patient communication and documentation through the Epic electronic health record
- Support quality improvement, patient safety, and patient experience initiatives
- Help improve patient access to cardiovascular specialty care and reduce wait times for consultations and follow-up appointments
- Work within a collaborative, team-based cardiology practice focused on delivering exceptional patient outcomes
Requirements
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current California Nurse Practitioner or Physician Assistant license
- DEA Controlled Substance Registration Certificate (CSRC)
- Basic Life Support (BLS) certification
- Advanced Cardiac Life Support (ACLS) certification preferred
- Additional certifications as required by the department
- Prior cardiology experience strongly preferred
- Experience in non-invasive cardiology, cardiovascular medicine, internal medicine, or specialty practice setting preferred
- Experience evaluating and managing patients with cardiovascular disease across the continuum of care preferred
- Demonstrated ability to work effectively within a multidisciplinary clinical environment
- Strong communication, patient education, and care coordination skills
- Experience utilizing Epic or similar electronic health record systems preferred
