Sutter Senior Care is seeking a full-time Nurse Practitioner or Physician Assistant to join our geriatrics-focused primary care team serving complex Medicare patients.
Location: Burlingame, CA
- Collaborative, team-based care model working in dyad partnership with a geriatric physician
- Focus on comprehensive, relationship-based care for older adults with complex medical and social needs
- Blend of direct patient care (~60%) and care management (~40%), including panel management and care coordination
- Opportunity for in-clinic, home-based, and virtual care delivery
- Interdisciplinary collaboration with RNs, social workers, pharmacists, and specialists
- Mission-driven organization focused on improving quality of life for seniors
- Provide comprehensive geriatric assessments, diagnoses, and treatment plans
- Manage acute, chronic, and complex medical conditions, including multimorbidity
- Deliver preventive care, screenings, and medication management (including polypharmacy monitoring)
- Perform in-basket management, including patient communications, results review, and follow-up
- Support transitions of care (hospital/SNF follow-up)
- Lead proactive panel management and care gap closure efforts
Requirements
- Minimum 2–3 years of clinical experience preferred; geriatrics or complex care experience strongly preferred
- Current California State NP or PA License
- CAPA Controlled Substance Certification (for Physician Assistants only)
- Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only)
- Nurse Practitioner furnishing number of NP’s
- Active DEA registration
- Experience in primary care, geriatrics, or value-based care models
Preferred:
- Experience with high-risk senior populations or panel-based care
- Familiarity with Epic or other EHR systems
- Experience with care management, transitions of care, or home-based care delivery
